It really depends on the restaurant.
5-10 years ago, a pint was a proper British pint of 570 ml, though almost no one ever used that measure outside the military circles. A pint meant a glassful (anywhere from 400-470 ml or so, from what I have seen in south India) .
Nowadays in India beer is sold in 675 ml bottles. Recently they are available in 375 ml bottles. So almost always you are likely to be handed a 375 ml bottle if you ask for a pint of beer. A kingfisher beer costs around 25 rupees for a small bottle and amount 50 for the larger one . In New Delhi proper, the taxes are higher and it will cost more, so it is better to buy it in Noida or some other border areas.

Lager tops is a pint of lager that has been "topped" off with lemonade. I always found it best to put the dash of lemonade in the glass first and then pour the pint (or half pint).

There is no set size it is more of a tradition or cultural matter. However in the UK there exists legislation as to how much liquid should be in a pint or half pint of lager or beer. The head is not classed as part of this liquid limit and should be in addition to the liquid. Generally about a centimetre is acceptable.
