It all depends on what you mean by "what do they do". It also depends on what type on incontinence they have. Some use a pad that is placed in their underwear, similar to a feminine pad. On the other end of the spectrum they use diapers. Again it all depends.
Vinegar and water does well. However, if your cat is frequently incontinent and you have carpet and/or upholstered furniture, you'll need a better solution to the issue. When the urine soaks into the pad, there is no real way to remove the smell.

You are doing this action incorrectly. There is a small hole in the pad, look for it. This hole leads to the wadding UNDER the pad and this wadding is what holds the lighter fluid. The hole is there to wet the wadding which in turn will fill and make the pad wet. This is how you know the lighter is full.
