Catalina is one option). It's difficult, but not actually impossible, to swim there from the mainland... there's a group of lunatics^H^H^H^H^H^H^H^Henthusiasts that occasionally do so. I've never read the book, but from the cover and the blurb I'm guessing that it might be meant at least partially figuratively, that doing something difficult, dangerous, and potentially fatal would be like "swimming to Catalina".
