It is manditory that you wat until the dog is 12 week (3 months) before it sets paw on the ground. If you refuse to do so, your puppy can get a virus and die.

i think making a dog live outside is definitely abuse. so yes they can come, and go inside and out. so never make a dog live outside, especially really old dogs, or puppies. reasons: they can gather ticks with lime disease, or fleas, if their old they may suffer arthuritus in the winter. they may be very skittish and if their puppies. when they grow up, they will be very dominate.

It's best to water them in thoroughly before letting your pet out, especially if he/she is likely to try and eat them.
if the dog has to go to outside but has no way out it has to go some time...try to take all food and water from the floor and put it somewhere where the dog cant reach it after supper so that it has time to let you know it has to go outside before the night starts
put all of it's favourite toys and chews out there and basically try to do as much as you can with it outside and it will end up wanting to be outside
Depends on the law in your country but usually only once.
what I did with my dog is anytime he/she would look like they would have to go to the bathroom i would put them outside and once they went I would go outside and get them and give them a treat and praise them.

You could put your dog a warm cloth on him / her and quickly go outside and take a shower, if possible use hot water. Or use fire beside... :)
keeping a dog crated means that when you leave the house or at night put the dog in its kennel. it teaches the dog to not go to the batroom at those times; it doen't like having its bathroom in the same spot where it has to sleep. which means you need to let the dog go outside before and after it goes in its kennel.
