To receive a provisional driving license in Ontario one must be 16 years or older. One must pass a vision test and apply to the Ministry of Transportation of Ontario.
That depends on which country you're in ! Here in the UK - you must already have your provisional licence in your posession before you start learning to drive.

No- with your application they should do a background check and see the suspended license in Ontario. If they miss it you may luck out but legally you should not receive a license until the end of the suspension.
You can receive a ticket with a fine, you also may get the vehicle you are driving impounded.
