Usually deaths attributed to "smog" (inclusive of ozone), are caused by constriction of air passages. The heart rate goes up, the organism goes into stress... requiring more oxygen, but the inflammation does not allow more oxygen to arrive (or as much CO2 to leave). So they smother.
