I believe it would very likely become a hot war long ago, probably a conventional WW3. Nuclear weapons scared people a lot, but probably prevented another global war.
While it is believed that Israel has possessed nuclear weapons since 1975, they neither deny nor admit to possessing any. It's widely believed that they are a nuclear power, although they have stated they will not be the ones to introduce nuclear weapons to the Middle East.

Nuclear weapons do not deter terrorists and indeed the use of nuclear weapons appeals to terrorists. During the Cold War large superpowers acheived a stalemate since neither side wanted to start a nuclear conflict which could result in Mutual Assured destruction however terrorists have no such concerns.
Neither the US nor Russia makes new nuclear weapons. The US refurbishes existing nuclear weapons at the Pantex plant outside Amarillo, TX. Russia has a similar facility operated by a corporation called Rosatom, but I don't know where exactly. Other countries may be making small quanties of new nuclear weapons.
Nuclear weapons are tools. Tools can be neither bad nor good. The only things that can be bad or good are the people using them and the purpose those people use them for.

Neither the US nor Russia are making nuclear weapons of any kind at this time. Other countries however are making nuclear weapons; some are North Korea, India, and Pakistan.
During the Cold War aircraft carriers had a secondary mission of nuclear warfare and its attack aircraft were nuclear-capable. However, it was policy to neither confirm nor deny the presence of nuclear weapons aboard any naval vessel. Pres. George H.W. Bush cancelled the nuclear mission for all naval forces except SSBN submarines. Today there are no nuclear weapons and no nuclear-capable aircraft on our carriers.
