For most of us we wait until a check engine light appears and a code(s) stored in the ECM tells you which 02 sensor(s) is not functioning but, if you had the proper scanning device you can moniter the performance of each 02 sensor and determine if it is up to par or not before it sets a code.
As far as I know they can not be cleaned. Must be replaced.
The 02 sensors are in the exhaust pipes but don't know about the updraft sensors.

There are four.
Some are and some aren't. There are different variations of oxygen sensors. First you have narrow band and wide band O2 sensors, then you have heated and unheated O2 sensors. Unheated sensors can have 1 or 2 wires , while heated sensors can have up to 5. Also some sensors are specifically calibrated to the unit they are intended for, so unless you really know what you are doing, then no they are not interchangeable.
Mostlikely one of the O2 sensors take it into autozone if you have one and have them run the codes with a reader and they will let you know what the codes mean.

There are four oxygen sensors screwed into the converter assemblies. You need to know which one is faulty before attempting to replace them.

Don't know what your original reading was, but the O2 sensors before the cats moniter how the engine is running and the O2 sensors after the cats moniter how the cats are performing.
4, one in front of catalytic convertor, one behind, on each pipe. If you need one replaced, ask the worker at the auto parts store to read the code and he/she will know which one is bad by the code given.
