Resident evil 5 is the actual game. Lost in nightmares is the DLC (Down-loadable content) Also their stories are completely different. Lost In Nightmares takes place three years before the events in Africa where Jill and Chris pursue Spencer in his mansion once again. It is set in a very Resident Evil 1 sort of feel but with the same action style as Resident Evil 5.
