One way to identify problems with an information system is to review it on a regular basis, review how often people use it, who uses it? Is the information up to date? Can it be accessed easily? Once you have reviewed all of these factors it is then you can start to identify the problems for example maybe all the information in your database is out of date or perhaps maybe some staff find it hard to use the database and therefore need extra training on this.
