One respiration is equal to one fall and rise of a patient's chest. The total number of times a person breathes in sixty seconds is known as the respiration rate. Any change in the respiration rate of a patient signifies that the patient is undergoing some form of stress or that there is an underlying medical problem.
For assessing the patient this is what one should do:Explain to the patient what you are about to do – even if the patient is unconscious.Ensure the patient is comfortable.Make sure the patient is as relaxed as possible.Observe if the patient is distressed in any way.It is best to monitor and record the respiration's immediately after taking the pulse – this will aid in a more accurate recording, as the patient will not be aware that you are observing respiration's. Awareness that respiration's are being recorded can make people alter their breathing.Observe the rise and fall of the chest (inspiration and expiration) - this counts as one breath. The respiration's should be counted for a full minute in order to have an accurate recording.Note the pattern of breathing and the depth of the breaths.Document your findings on the patient’s observation chart, note any changes and report to the medical team. Before leaving ensure the patient is comfortable.
It depends on the circumstances. They may be psychologically unstable - and try to harm themselves. The might fall and injure themselves, they might try to attack another patient...

when they fall through the floor.
You can count their respirations by counting each chest rise and fall. You can watch a lot of other appearance factors such accessory muscle breathing. You can observe their skin color and condition as well.

Most patients with ABPA respond well to corticosteroid treatment. Others have a chronic course with gradual improvement over time. The best indicator of a good prognosis is a long-term fall in the patient's IgE level.
