We do not know of any planets that have been destroyed by a black hole, but it probably has happened. Because of the vast distances in space and the fact that neither planets nor black holes emit light, both are hard to detect.

No. You know that the stronger the gravity of a body is, the faster you have to move in order to escape from it. The gravitational field of a black hole is so strong that light can't escape from it. And as you know, light moves at the speed of light.

Because there is usually nothing around the spot where the black hole is because the black hole "ate" everything.
We know nothing about the conditions within a black hole, but it seems unlikely that a black hole could exist within a black hole, or even if this concept would have any meaning at all.
