At 18, you're a legal adult. You don't need any forms; as an adult you can take over any personal matters you want. You should know, though, that it takes a minimum of $20,000 a year to maintain a decent household, and that's just above the poverty line. A minimum wage job pays about $8,000 a year after taxes.

You are a legal adult at the age of 18. You can legally move out. But if you're just in a fight with your parents, I'd suggest hanging in there! It's not worth jeopordizing your education - and without a good education - you can't afford to move out anyway!
