If you pour water into the radiator and it immediately pours onto the ground on passenger side of 95 windstar can i assume that the water pump needs replacing?

Try get a better look at where it's leaking from, it might very well be another component that's broken. Could be the radiator even.
Yes, a broken hose even could be a likely possibility, and a whole lot easier to repair.
