The first shuttle to explode during a mission was the space shuttle Challenger back in 1986. It exploded 73 seconds after launch, killing all 7 people aboard. Space ShuttleEndeavour was built to replace Challenger.The most recent shuttle disaster was in 2003 with space shuttle Columbia. The shuttles heat tiles were damaged during launch. The damaged tiles were not able to protect the shuttle effectively and exploded killing all 7 astronauts aboard.
What made Sally Ride well known is that she was an American civilian school teacher who was aboard the space shuttle called Challenger, which exploded 76 seconds into its flight, killing all aboard. As a civilian, her participation in the Challenger flight had been heavily publicized and more than the usual number of people were watching the lift off at the time. All seven people aboard perished in the Challenger Space Shuttle in 1986.

Columbia didn't explode at lift off. I think you're mistaking it for Challenger, which exploded early in it's flight. Columbia broke apart as it was re-entering the atmosphere. there was a piece of foam that broke off early in the flight (within the first 90 seconds I believe they said) and it doomed the shuttle. the Year it xpolded was 2003 it exploded at lift off 7 people died the Year it xpolded was 2003 it exploded at lift off 7 people died
