In the history of the shuttle program, no shuttles have ever crashed. However, two Orbiters were lost due to catastrophic malfunctions. On January 28, 1986, 73 seconds after the launch of its 10th mission, the Challenger, STS 51-L exploded killing all seven crew members. On February 1, 2003 near the end of its 28th mission, the Columbia, STS-107 disintegrated upon re entry, killing all seven crew members.
