This is only a guess based on observation. If anyone has read how this was lit, feel free to correct as necessary.
It looks to me as though the main light source is a lamp up high, to the viewer's right of center, pointing down about 60 degrees off of vertical and behind the near/far center point of the subject. I also think there was secondary lighting; probably a soft reflector in the foreground to the right of center.
