You can add it on the Insert tab. When you are setting it, there is an option to have a different first page for footers. So you can set it to be on the first page, and not on any other pages.
There is no simple answer to that. It is up to you what way you want it. In different situations you may even want to have it different ways. Details that you might want on time may not be needed on another. The size, colour and fonts you use might be different in different situations. You may have it done differently if there are a lot of pages in a worksheet than when you can print it all on one page. So the footer should be formatted the way you want to format it in each situation.

In MS Excel, each page is called a worksheet.
It probably depends on what version of Excel you are using, but on my Mac, I would go to the Layout tab and select Header & Footer under Page Setup. Then I could add things on the resultant window.
