When the car stops suddenly (like if you slammed on the brakes to try to avoid a collision) inertia keeps the people moving forward. The seatbelt would have slammed into the person pretty hard, and the sheer force would cause the bruising.
Because whena car suddenly stops, you tend to get thrown forward a bit, and the seat belts prevent this by locking the belt so you don't go forward. And it tightens the piece around the waist too. If you squeezed you abdomen hard enough this would happen.
