This can happen to many people. If you earn it, but you are not a Spartan Officer, you may have to become a Spartan Officer. That or sometimes armor can get glitchy and will eventually come back. It just takes patience. It's ok, this happened with my ODST helmet.

yes you can lose armour through halo it happens with patches and fix and i remember when everyone lost kantana and secruity armour you can also unlock armour for nothing through bugs hope this helps

What do you mean - Halo 3 or ODST? If it's Halo 3 then: What armour are you talking about? The shield? Or HEAVY ARMOR VEHICLES? If it were the shield then yes. You stay in a safe spot and wait for several seconds. If it were the vehicles, then bad luck (if it blew up). ODST: Get a health pack.

The profile is your gamer profile. If you are not using the gamer profile you had your halo stats on, there is no way to transfer them. If you are using the same profile, your data is lost.

Okay, you didn't do the 49 original Halo 3 achievements. You also need at least 1000 Gamerscore in Halo 3 alone. So if you've met those, go to Bungie.net and then send them a message saying that, and they'll re-activate it. If/when you get it back, don't do any more of the original Halo 3 achievements. The original ones are the ones that are done on maps, or offline play, that aren't on mythic maps, just so you know. ;)
