Dream is defined as a process which takes place in a mind which can effect the reality. It is well defined in the movie.
The totem is a device that Cobb spins in order to tell whether he's in a dream or not. If it spins perpetually, he's in someone's dream. If it spins, but then falls and stops, it's reality.

This question is not entirely clear. Persons do not "get stuck" in dreams, because dreams end when the dreamer awakens. The idea that one can "get stuck" might have come from science fiction such as "Inception." Such ideas are not based in reality.

But the ability to control dreams and intentionally (consciously) move from one dream into another then another, or to move in and out of other people's dreams as portrayed in the film Inception, is pure science fiction fantasy. No, the art of inception is a fictional concept. No one can get into anyone else's dreams - not even those who make claims about "dream walking." Dream invasion is a fantasy of science fiction.
