The Franciscan order, primarily, were the missionaries who established many missions in the Southwest and in Florida. They frequently traveled with the Spanish conquistadors for that purpose. The Jesuits also established a number of missions in the Southwest. Padre Kino was a Jesuit who established a number of Missions in California. Padre Junipero Serra was a Franciscan who also established missions in the Southwest U.S.

It depends on what kind of mission you're talking about. If you're talking about LDS missions, it varies depending on the country, state, population, and need for missionaries. It can be a city, a county, a state, or an entire country. Some missions even covered several or parts of different countries. Missions can overlap the boundaries of the land, so its not restricted to just one place. It could be several neighborhoods and a half if that's what God needs. Personally my mission covered the southern half of one state and the eastern half of another, all Spanish speaking. It was quite the experience.
