There is a 10% chance that your sim will have twins. But if you really want them to have twins then do Ctrl + Shift + C and a cheat box will pop up at the top of your screen and then type in "ForceTwins". If that doesn't work then you have to type in the cheat box "boolprop testingcheatsenabled true" then select the pregnant sim then Shift + Click on that sim and go to Spawn>Tombstone of L and D and a tombstone will pop up next to your sim click on that and it will say something like "DEBUG - ForceTwins" and click on that and then your sims will be pregnant with twins! After you have done this the next time you want to make one of your sims have twins then you can type in "ForceTwins and it will work. I hope that helped.
