Sora is born on what seems to be a relatively normal world. Over the summer he heads out and lives on Destiny Islands. You can see the world outside of Destiny Islands in Kingdom Hearts II when Kairi is writing her message in a bottle.
You can not play in Destiny Islands, like in almost all other Kingdom Hearts games, however, many important cutscenes do take place in Destiny Islands.

Yes,
Yes. Sora and Riku both visit Destiny Islands later in the game. Sora visits the Destiny Islands world card and finds the truth about Namine and kairi, after defeating a Darkside Heartless. Riku visits Destiny Islands when Zexion creates his own version of Destiny Islands and tries to defeat Riku using an illusion of Sora. Zexion (as Sora) tells Riku that he is a dark monster and that the light will destroy him. Riku is on the verge of giving up, when Namine (pretending to be Kairi) convinces him to use both darkness and light).
IT is not clear that "destiny" is real. However, if it is then logically it could not change.
