The Doctor (of Doctor Who) is currently a male. He regenerates when his body is damaged severely enough to a point near to death and so assumes a new form. This has, so-far, resulted in him assuming only male forms, but it is assumed that he could assume a female form during this process (in the second part of The End of Time, when he regenerated from the Tenth to Eleventh Doctors, after feeling his long hair he expresses something along the lines of: "I'm a girl!" So it may indeed be possible for the Doctor to become a female.
Again, if we are talking non-canon stories (i.e. things that happen outside of the regular Doctor Who chronology) then the Doctor did regenerate into a female form (the Thirteenth Doctor) in the Red Nose Day spoof Dr Who story "The Curse of Fatal Death," broadcast on the 12th March 1999, and was played by the fantastic Joanna Lumley.
