Compare this with PRK where the same excimer laser is used but rather than treating under a flap like in LASIK, the treatment is done on the outer surface of the eye after the epithelium or skin is removed. Here in PRK there is a large wound which must heal and it is painful, the vision is blurry, and there is a much greater chance of scarring. The latter problem has been greatly reduced to some extent with better lasers, but mostly with the use of Mitomycin C. This is a chemotherapy medicine which stops the proliferation of fibroblasts as part of the healing response. The use of Mito C is "off label" and not endorsed by the FDA although it is quite effective. We believe that this is a safe process, but there is nothing studied to prove that belief.
The benefits of PRK are that it is more superficial and thus less likely to cause as much weakening of the eye as LASIK, although with the newer blade free treatments this is less of a difference than before, and it does not require making a flap which has some inherent risk.
