Ezio did not die in any of the games, Instead, he dies in a short film by Ubisoft called Assassins Creed Embers.

The next portion in the AC universe after Revelations is Assassin's Creed: Embers, an animated short film made by UbiWorkShop (a division of the game's maker, Ubisoft). Embers chronicles Ezio's final years in a villa outside of Florence, where he meets a young Chinese assassin who travelled a great distance to find out how she could rebuild her order, as Ezio had so skilfully done in Rome. AC:E can be purchased for 240 Microsoft Points on Xbox Live and for $3.99 on the PSN (PlayStation Network).
No one knows exactly what happened to Ezio. When Assassin's Creed: Brotherhood ended, there was no mention of his fate. Curiously, there is no document or record about Altair's fate either. It is possible (and likely) that Ezio gathered his forces across Europe and continued to fight the Templar Order up until his death; when he died however, is not known.
