Great whites aren't exactly cold water sharks although they do live in the colder waters (such as Vancouver island area) but they usually go to the warmer areas of the coast for winter (such as California :D )
There has never been a documented case of a Great White shark in freshwater, although they were blamed in the New Jersey shark attacks in 1916. This was long before shark researchers had any idea about the fish, and scientists now believe it was a bull shark that was the culprit.

No. Perhaps at the mouth of a freshwater river that empties into a salted sea, but not in strictly fresh water. The Great White shark can survive for a limited amount of time in such brackish conditions. However, it will seek salt water and will not stay for long. Bull sharks can swim in fresh and brackish water, though.
Most definitely. Nearly all sharks are saltwater, though a few (bull sharks and nurse sharks most notably) can inhabit brackish water (with some salt content) in rivers near to the sea.
