There is no information on when or whether they will release the maps that were included in the Hardened and Prestige Editions of Call of Duty Black Ops. From what I can learn the Maps are only similar to the Maps in the Map Packs from Call of Duty World at War and those Maps do not and will not work with Call of Duty Black Ops. The Maps also appear to have been part of the game disc and not a download someone can sell of give you a code to. The hardened and Prestige Edition were only available for the PS3 and Xbox 360 game consoles and were not available for either the Wii or DS Nintendo Call of Duty Black Ops game versions The rezurrection Map Pack for Black Ops includes the 4 WAW zombie maps and a new Zombie map Moon. It is $15 or free for owners of the Prestige and hardened editions of black ops
You buy it with the rezurrection DLC and get 5 zombie maps for $15. It was never a code except for a one time use code that went with the Hardened and Prestige Editions of the game
Just because they used the maps from world at war in black ops prestige and hardened editions of Black Ops does not mean they will continue to do so for Black Ops II. You should definitively not expect it to be part of a standard edition of the game
You can not. There are no Map Packs for the Call of Duty Black Ops Nintendo Wii version of the game. The Nintendo Wii Call of Duty Black Ops was also not available in the hardened or Prestige Editions of the game with the maps from Call of Duty World at War because those maps were never available for the Nintendo Wii. The Hardened and Prestige Editions of Call of Duty Black Ops were only available for the Xbox 360 and PS3 because they were the only consoles that had Call of Duty World at War Map Packs 1 through 3.

Because it is a new game that cost less than the other editions. It's like asking why yours did not come with the RC-XD Video Surveillance Vehicle or any other features that went with the higher cost Hardened or Prestige editions of the game
If it's new, yes. Ebay has information with the used games for the hardened edition saying that the code was a one time use code. If the game has had the code used the maps are not available. Many of the hardened editions that are new and being sold on Ebay are the prestige edition games that are being split up and sold separately
Hardened edition of black ops costs a little bit more but in the hardened edition you get the following things A small black ops medal The 4 coop zombie maps from call of duty WAW and the game comes in a steel case in a small box packaging unlike the original game. Hope this helps :)
The codes come with the Hardened and Prestige Editions of Call of Duty Black Ops and are a onetime use code that can not even be made to work a second time if you purchased a used Hardened edition of the game. You must find a new copy of the Hardened or Prestige edition of Black Ops if you want the original Maps, which are collectible and very expensive
No it was the Black Ops Hardened or Prestige edition that had the 4 World at War Zombie Maps or they were also in the Rezurrection DLC for the first game
