Yeast causes both physical and chemical changes to bread dough. It digests sugars which produces gas (chemical changes,) which in turn lifts the dough's mass (physical change.)

No, because the dough is a solid, and the bread when it's done is a solid. But if you cook the bread too long at a high temperature, it will turn to ash, which WOULD be a physical change.

A chemical change because it causes the bread to rise. Also, the yeast can't be changed back into its original form after the change took place.
