Usually all caramel is just water or milk, and sugar. You could use other things like corn syrup or even brown sugar, but traditionally it is just you liquid and sugar and a soft heat source and constant stirring or your sugar will just scorch and not caramelize.
I'm sure you could add caffeine crystals to it, and I am sure it has been done. You could add a lot of things to it if you wanted. You would add it towards the end though. Caffeine does break down at higher temperatures. In the case of coffee, if you have a lighter roast, you have higher caffeine levels. If you go for a darker roast like say, French roast or espresso, you are getting way less caffeine and more burned flavor (*cough* Char-bucks)
You would want to add the caffeine after the mixture starts to cool down. It will happens pretty fast and you had better have your destination for cooling already setup. Mix it in and pour it out.
Although, I don't really see the benefit of adding it, caramel is wonderful and complex all on its own.
Caramel is made from sugar and cream ( one of the best ways to make caramel is to boil sweetened condensed milk in its can for about4 hours. yum ;D) So i would probably say, no on the caffeine.
