From my personal expierience, yes it is. I found out I had genital herpes in April and I had health insurance, but lost my job a week later and the insurance was dropped. Now that its August, I tried to get health insurance with FL BCBS and after I paid them (conveniently), they sent me a letter saying they can't cover me due to a pre existing condition. The only "condition" I have is genital herpes. So I would tell you yes they do.
Any medical condition that happens prior to you applying for the medical insurance is going to be considered a pre-existing condition. If you're applying for individual insurance, most likely the condition will not be covered but possibly medication for the condition would be covered. If you're applying for group insurance, you can check with the benefits administrator at your job to see how pre-existing conditions are covered in the policy, reason being because individual and group policies differ in coverage.
You are thinking that some conditions are considered "pre-existing" and others are not. That's not it. Did you already have the condition before, say, applying for insurance? That's the idea of pre-existing.

Yes, it will be pre-existing. Probably what will happen is the insurance comapny will place a "rider" on your back. The rider will exclude coverage for any condition related to your back, and may even list specific areas of the back that would not be covered for any future complications. Some companies will still pay for the medication (if any).

If one was applying for health insurance and had to list existing conditions and ADHD had been diagnosed prior to the application it would be pre-existing. If it was diagnosed after the application and after the insurance is in place it would not be considered pre-existing, even if it did exist, so long as it had not been diagnosed by a doctor. Yes. It is a guaranteed issue policy so therefor you cannot be turned down. Also depending on the condition you may be able to still obtain a regular fully underwritten policy. You should speak with a knowledgeable broker that handles both rather than just blindly applying for insurance.
