It's hard to answer this without more information. How long ago did you breakup? Have you tried to talk to him and did he tell you that he wanted to be left alone?
Assuming you broke up with him recently, and if the break up was bad, i.e. he was heartbroken, maybe it was over another guy...etc. I think all you really need to do is call him and ask him to meet up with you to chat--if you have similar friends go to a party where you know he will be--this is really cheap, but if your stuck-- tell him you want those dvds you left at his place--basically do whatever you can to meet with him face to face. If he initially blows you off--don't be surprised, he's probably hurt. Maybe try one more time but then back off--you don't want to push it.
If you are lucky to get him to meet up with you. Just tell him you are sorry. Tell him you were scared of where the relationship was going--or whatever the issue was. Tell him that you would like to date him again--but that if he needs sometimes to think about it--you'll understand. Depending on what happened between you guys--he may need sometime.
Finally, one word of advice: do your homework. Try to find out if he has a new girlfriend first. Your confession coud be quite awkward if he is already with someone new. It's just not something you want to be surprised with without being able to get yourself together first.
