Most likely were you too dry, there was chafing or he hit just a little too hard too deep. It takes about 3 days for the egg to get fertilized though and another week to get attached so it has nothing to do with conceiving.
Yes. I had alot of bleeding at about 8 weeks pegnant. I thought I had lost the baby a few weeks later I went to the doctor because of stomach pain and it turns out I am still pregnant.
