Sakura still has feelings for Sasuke yes but does not see Naruto as a sibling. Sakura has flirted with him before and offered to feed him once(which is a romantic gesture in Japan) and hugged him twice in public(which Japanese people don't normally do.) But, to be technical, Sakura doesn't know what she feels for Naruto hence the confusing, not 100% geninue confession. She obviously loves him, just doesn't know that yet.
