Generally 4" soil pipes are used for the runoff of toilet waste. A 4" waste pipe has got to be installed to the back of a toilet. Toilets are made with standard outlet of 4 inches, thus ensuring that a 4" pipe is installed because a sewer line in the main line is 4 inches, and it makes no sense in reducing the pipe to three inches, only to have to enlarge it again so that it can join up to the main line. The reason for it being 4 inches is to prevent blockages, because a vast amount of water is running through the pipe along with toilet paper and sewer, so reducing the line will increase your chances of a blockage.
The average size of a toilet is 21-30 inches wide, 14-16 inches from bowl to rim, and has a depth of 27-30 inches.

The width of a toilet tank can vary in size. The average width of a toilet tank is about 18 inches.

Probably it is due to the size of the toilet tank. If the toilet is a low flow, or under 2 gallons per flush, the tank will be considerably smaller than an older toilet tank, which might hold as much as 25 gallons. The only critical thing in istalling a toilet is if the drain pipe lines up with the drain opening. Unless you move the toilet drain, the new toilet is going to sit in the same place. If you have a gap between the wall and the tank, you might want to put a block of wood or something to help anchor the tank. Newer tanks are smaller due to the reduced water need with the current flushing technology.
Assuming you have a standard toilet now, it will bolt on the same way in the same position. ADA are just taller floor to seat. Most toilets are 12 inches from the back wall to the center of the drain. Older ones, before 1940 or so were 14 inches and I have come across a few odd ones that are 9 inches.

Not all toilet papers are that size. That might be the standars size because the tissue holders are that size as well. But there are othere kinds of tissues which are different in size.Toilet paper is a standard width so it will fit in toilet paper holders.
