No, but it has a condition attached. It's not enough to just go back and redo missions to get no kills, for example if you played missions 1 to 9 but killed someone in mission 4, you would not get the achievement if you redid mission 4 with no kills, even though now all your stats show as no kills.
What you have to do is play through each mission in turn and get no kills. So in the above case you'd have to play 1 -3, then when 4 says you killed someone, redo it without advancing to 5, and then do 5 - 9 without killing anyone. Only then will the achievement be granted.
No one. The two assassins you fight are illusionary distractions created by Daud. They disappear when killed rather than leaving a body (remember the plan is to frame Corvo) and don't count against you for the Clean Hands achievement.

It depends. are your hands dirty or clean? Even though you have clean hands, the bacteria swaying in the air catch hold on your hands. So right now, you might approximately have 7,000,000 bacteria on BOTH of your CLEAN hands!
Maybe your hands are nasty and its trying to clean the filth from your hands off of its partially clean body ...

Baby wipes are made to be soft. They clean babies, so it seems logical you can use them to clean your hands.
I wouldn't suggest it but if you did it you're hands wouldn't get clean. They'd get wet, but not clean.
Before you point your finger make sure your hands are clean
