I don't know of a company that will do this for you, but you can make the pattern yourself using basic photo editing software. Check out Google's free image editor, the Gimp. All you need to do is decide the size you want the finished pattern to be, divide by how many stitches you have in an inch, and then overlay a grid with that many squares onto your image.
For example, say you wanted your finished item to be a 10 inch x 10 inch square. If you have 5 stitches per inch, you'd need a grid that was 50 squares x 50 squares. Just overlay that grid onto your photo, and decide the dominant color for each square. That color will be your yarn color for that stitch. The smaller your stitches, the less pixilated the finished item will look.
Then you can go to a craft store and buy canvas and yarn in the colors you need, or you could dye the yarn yourself.
