You describe a very complicated set of symptoms, and even when the symptoms are fewer and/or less complex it can be a process to come to the right conclusion. I doubt that there is one single DSM diagnosis that would adequately cover everything you mention. I think there would be a cluster of disorders, from the current categories of the question, that might cover most of them. There could also be some psychotic involvement.
