A scientist may create an hypothesis on the basis of their observations. If their observations are carelessly made or recorded then any hypotheses they offer will be inaccurate reflections of nature. A scientist may alternatively collect observations to test an hypothesis. If these observations are carelessly made or recorded then the conclusions that the scientist makes about the hypothesis could very well be incorrect. At the very least the conclusions they draw would not be based on sound evidence (because the observations are unsound).
It means that the conclusion that you draw is a result of your observations. So, if you observe a bear hibernating, you might conclude that bears sleep a lot. If you see one mauling a deer, you might thing that they are dangerous predators. Both are actually correct, but you draw different conclusions based on what you observe.
All modern models based on science that are worth their salt would be. The only major theory I know about the origin of the universe is the Big Bang Theory, which is supported by Red Shift observations. Galaxies we can observe are all moving away (we know this due to Red Shift) from a central point, believed to be the epicenter of the Big Bang.
It is true that scientific models are based on a set of observations, along with a logical analysis of those observations.

Inference is based on drawing conclusions or making predictions using reasoning and evidence from given information or observations. It involves taking what is known and making educated guesses or interpretations about what is not explicitly stated.
An explanation of things or events based on many observations is a theory.
Quantitative observations are observations that can be precisely measured. Qualitative observations, meanwhile, are subjective observations that are based on the characteristics of what is being observed.
