The coolant fan may have a blown fuse. The electric motor, on the coolant fan may be not working. The ground wire on the cooling fan may be corroded or loose.
12 speed, you mean 1 or 2 speed? Anyway, when a fan speed control only works on high it almost always means the Resistor Pack is defective and needs replacing. It can be the speed control itself but it is highly unlikely. 99% of the time it is the Resistor Pack.
I have a '99 Cirrus, where both the heater and A/C did not work - I ended up having to get the fan engine and rad changed out - $1500 I believe....
The big fan that is on the front of the engine is bolted to the water pump.

The heater fan is behind the glovebox. If you remove the glovebox and look up from underneath, you'll see the blower motor. It is held on by three screws. In front of it the blower motor resistor, which also sometimes goes bad.
Under the hood, on the passenger side of firewall, near the blower motor.
on my 99 Sephia the Blower is located under the dash on the passenger side.

99% of the time it is the heater resistor block, it is a bunch of resistors which gives you the different speeds. this is mounted close to the heater motor, pass side of the car. Usual cause of failure: water or snow inside the heater housing.
