The Miranda Rights declare the right to remain silent and the right to an attorney. They are no the source of these rights. These rights are outlined in the 5th and 6th amendment.
Many police officers carry a card or a notebook containing a Miranda warning ("you have the right to remain silent," etc.), so they can testify in court to the precise language they used when they gave the admonition to a suspect. It isn't done all that often. In most circumstances where a Miranda warning is administered, the warning is on a paper form that the interviewer and suspect read together, with the suspect initialing each paragraph and signing at the bottom of the form. This form is then made part of the case file. If the suspect later says he was not properly warned, the form is used to impeach his claim.
The term "Miranda rights" comes from the 1966 case Miranda v. Arizona. This case determined that a confession obtained by rough interrogation was inadmissible. The rights that Mr.Miranda should have been made aware of were compiled and now must be read to suspects before questioning.
