One suggestion is to talk to the end-users in the beginning. Too often something gets developed without ever communicating with that level. Another suggestion is more planning, and more checks. In many companies, the initial design of something is based on just a few people. Get more people involved on the front end, and the end result will be better... plus, if you publicize the plans, and share them with more people, you might be able to catch more problems early. More testing, at earlier stages, also helps. You will also get more meaningless feedback... but I feel it is worth it. Hard to do though... have to fight against stagnancy and politics and "need to know" policies in order to get more people involved, especially ones who have no power in the context of a company.
Typically it is Applications software, so things like word processors, spreadsheets, databases etc. It could also be software that has been specifically written for a user for a specialised purpose.
