The fan only comes on when the thermostat tells it things are hot. That means either things really are hot, or the thermostat is broken.
So you might have to replace your thermostat (or test it). It is cheap and easy to change out the coolant (flush and fill) so you might give that a try but it probably won't make any difference unless you notice first that the coolant level was low. Use replacement coolant that is rated for 150,000 miles (or more).
Other things can contribute to radiator to heat - it is basically air cooled, that means air needs to pass over it as efficiently as possible. Look for any obstructions in your front grills and in the radiator fins. These areas can easily get plugged up with leaves and road debris.
