This would be much easier to diagnose if you can get the car to the auto parts store (ie PartsAmerica or Autozone) and have them run the codes, then search the forums on Taurusclub.com to see what others have found.
Otherwise, most likely culprits (in order):
EGR valve
IAC (intake air control)
DPFE - Differential Pressure Feedback EGR
This is, of course, not everything that could cause the issue. Get the codes checked and you can find out for sure.
