There have been two U.S. Vice Presidents appointed and confirmed according to the 25th Amendment to the U.S. Constitution:
Gerald R. Ford was appointed by Richard Nixon to replace Spiro T. Agnew. Ford's appointment was confirmed on 6 Dec 1973.
Nelson A. Rockefeller was appointed by Gerald R. Ford to replace himself. Rockefeller's appointment was confirmed on 19 Dec 1974.
In 1973, Vice President Spiro Agnew resigned from office in order to avoid criminal charges for bribery. According to the 25th Amendment when there is a vacancy in the vice presidency, the president would nominate and the Senate and House would confirm a new Vice President. Gerald Ford was chosen as a moderate. He took office on December 6, 1973. When Nixon resigned on August 9, 1974, Ford became the first individual to be President who was not elected to either the presidency or the vice presidency.

Gerald R. Ford was the vice-president appointed after Spiro Agnew resigned. He is the only vice-president appointed into job. Oddly, he became president after Nixon also resigned his office.

Under the 25th amendment of the U.S. Constitution, a new vice president is nominated by the president and confirmed by a majority vote of both houses of Congress.
After Vice President Spiro Agnew resigned in 1973, Nixon nominated Gerald Ford to replace him, as required by the 25th Amendment, Section 2. After President Nixon resigned in 1974, Gerald Ford became President, satisfying Section 1 of the 25th Amendment.

When vice-president Gerald Ford became president due to the resignation of President Nixon, Ford, acting in accordance to the procedure for filling a vacancy in the vice-presidency, nominated Rockefeller for the position and Congress confirmed the appointment.
Amendment 25 sets rules for temporarily giving the vice-president the powers of the President, both by request of the President and involuntarily . It also gives a process for restoring the president's powers. (The main body of the Constitution makes the vice-president the president if the president dies or resigns.)
Under the 25th amendment section 4, the Vice President makes the determination. He makes this in writing to the President pro tempore of the Senate and the Speaker of the House of Representatives in writing.
