Use of a "safe" weedkiller which was not safe for your type of grass
A stone or piece of concrete preventing root growth there
A critter eating the roots from under the lawn in that spot
Lawn beetles eating the root mass often form near perfect circles
Drip from a very acidic tree which concentrates on one spot.
