They pop off. You use a screwdriver or some type of panel removing tool.

There is a special tool for the removal of the window crank handle. This item costs less then $5.00, you can search for it online.
Manual window cranks are held on with a screw on 1997 Honda Civics. Remove the plastic cover on the handle to reveal the screw. Loosen and remove the handle. Replace with a new unit.
Its probably the window regulator inside the door. Most of them have nylon gears which get brittle and break. You can get to it by removing the interior door panel. From there you should be able to determine how hard it is to get it out.

On many occasions the plastic inside the handle strips out and the handle just spins. I'm assuming you are talking about the interior handle possibly the manual window handle. If so, you need to push on the area surrounding the handle to expose a small clip. use a standard screw driver to push the horsehoe looking clip to remove the manual window handle. To remove the arm rest there are two hex screws on either side..they are somewhat hard to find. You can then take steps to remove the inner door moulding.
