To replace a lost Florida concealed weapons permit, you will need to submit a completed application for a duplicate permit to the Florida Department of Agriculture and Consumer Services along with the required fee. You may also need to provide a copy of a police report if your permit was stolen. Contact the department for specific instructions and to obtain the necessary forms.
it depends on what state. Here in Tennessee, you simply go to the driver license testing center and tell them you want to apply for a duplicate permit. It costs $5 and you receive it in the mail a few weeks later.
