Not cleaning the pipe or joint enough, not using flux, or not heating the pipe or joint enough. You use the blue tip of the flame on the joint and the capillary action sucks the solder into the joint. You have to clean the pipe with sandpaper or steel wool, the joints with a fitting brush on the inside, and use a proper flux and solder.

How can use instead of Teflon Tape to seal the 4 threaded joint on galvanized air lines?
You can use pipe thread sealant aka "Pipe Dope" you can get this at most hardware stores.
Why does spout leak at the wall when shower diverter is used?
A hole has developed in the spout or where it screws onto the pipe needs more Teflon tape or joint compound to seal the threads.
What type of pipe thread sealant should you use?
Teflon tape is good. There is now a teflon liguid sealer also. The old pipe sealer is still good. I used a piece of radio solder last year to seal a water meter joint when I lost the meter seal. Be inventive.
Can you seal a leak from cemented pipe elbow or should you dismantle and start again?
Yes you can seal a leak from cemented pipe elbow. I tried this few times and it works for me. If you can seal somehow temporily seal one end of the pipe, I use packing tape. Seal a the shop vac hose onto the other side of the pipe. The stronger the vac better. Turn the vac on, it will draw air from where ever it can. This will be at the leaks of course. Apply a generous amount of cement completely around the leaky edge of the pipe. The vacuum will then, draw the water out and the cement in. Then allow sufficent time to cure. That is just a temp cure. You don't want to dig again to get to the joint. Remove and replace. k
